What Information is Needed to Start my Adoption Application?
If you would like to adopt a child, then the following document may be required:

Registration of Birth,

Birth Certificate,

Family and Medical History,

Death Certificate (of any deceased parents or guardians),

Criminal Record Check,

Home Study Report, and

Cultural Connection Plan Form.
